The translational process of new diagnostics from research and innovation to adoption and implementation into the health care system can be time consuming, resource intensive and hold considerably high risk of financial loss (Ijzerman and Steuten 2011, Price, Wolstenholme et al. 2017). Cost effectiveness analysis typically occurs in the late-stage of diagnostics development when significant resources from the industrial developer and/or government funding agencies have been invested and otherwise wasted if regulatory approval cannot be obtained. In the last two decades, the concept and application of model-based economic evaluation during the early stage of health technology development has been discussed and is now implemented collaboratively by industry, government bodies and academia to help select and harness the development of emerging medical technologies (Abel et al. 2019). However methodological research into the development and implementation of early-stage economic modelling has to date been largely conceptual and exploratory in nature.

Building on previous work and collaboration with NIHR Community Health Care MIC in the department, this doctoral proposal aims to establish a methodological framework for developing and implementing early phase economic modelling for diagnostic interventions to guide their further development and adoption, specifically:

  1. To review, examine and evaluate current methods used for developing and implementing early phase economic modelling for diagnostic interventions;
  2. To review and establish clinical care pathway analysis to inform early-stage diagnostic modelling;
  3. To determine a standardised method of quantitative expert elicitation that can be used to inform diagnostic specific parameters for early-stage diagnostic cost effective modelling;
  4. To involve experts and stakeholders in the process of methodological framework development and dissemination.
