Our ambition with Connected Care is to reduce hospital use by supporting individuals to manage their respiratory health effectively at home. By harnessing digital technologies and personalised data, we aim to:
Enable early detection of health deterioration through remote monitoring and real-time alerts.
Support people with self‑management tools that build confidence, insight, and proactive care routines.
Reduce hospital stays and facilitate safe early discharge.
We believe that digital devices, wearables, and secure data platforms and the insights they generate when used responsibly, can shift healthcare from reactive to proactive. This approach is designed to help people stay well, reduce risk, and avoid unnecessary admission. CARRii’s infrastructure across the UK - including unique population-scale health data platforms—provides the scale and insight needed to study and implement these innovations in real-world settings.
