This resource is designed to be used by GP surgeries to help signpost patients to local community groups that could support them with healthy living.

It was designed and co-developed by people living with obesity and researchers in the Health Behaviours team.

The suggestions of the types of community groups on the resource all came from people living with obesity based on their experience of what has helped them. However, the community groups may be helpful for a range of people who visit their local GP.

We hope that you find this a valuable resource, which can be filled in and printed off, as well as displayed on reception screens.

This PowerPoint slide is editable and can be updated with local resources, and shown on surgery displays, or printed as a poster.
